A PCN is a group of practices who work together to provide additional services and appointments to patients. The overall aim is to improve patient health, provide support and help to those in heed and to improve access in health care.
The practices in Salutem PCN are:
the additional roles which the PCN offer to our patients to allow patients to have better access:
work within the multidisciplinary team of healthcare professionals and support the delivery of safe, effective and efficient systems for medicine optimisation, repeat prescribing, reducing medicine wastage and maximising patient outcome. They are unable to prescribe medication.
they identify and resolve actual and potential drug therapy problems through provision of comprehensive medication management.
working collaboratively within the Multidisciplinary team to meet the needs of patients , presenting with minor aliments.
connect patients to local groups, activities and services within the community to meet the practical , social and emotional needs that affect health and wellbeing
